TLC Nursing Associates, Inc. is seeking an experienced Registered Nurse (RN) – Skilled Nursing Facility (SNF) to provide high-quality care to patients in a long-term care or rehabilitation setting. This role involves assessing residents, administering medications, coordinating care plans, and ensuring patient safety and well-being.

Responsibilities

Provide comprehensive nursing care to residents in a skilled nursing facility (SNF)
Administer medications, IV therapy, and treatments according to physician orders
Assess and monitor patients' conditions, reporting changes to physicians and healthcare teams
Collaborate with LPNs, CNAs, therapists, and other healthcare staff to develop and execute patient care plans
Document patient assessments, care plans, and progress notes accurately
Educate patients and families on disease management, rehabilitation, and wellness strategies
Ensure compliance with state and federal regulations, facility protocols, and infection control policies

Qualifications

Active Registered Nurse (RN) license in applicable state(s)
Minimum 1 year of experience in a skilled nursing facility (SNF), long-term care, or rehabilitation setting
Certification in Basic Life Support (BLS) required
Knowledge of geriatric care, wound management, and chronic disease management preferred
Strong communication, assessment, and critical thinking skills
