Responsibilities
- Provide comprehensive nursing care to residents in a skilled nursing facility (SNF)
- Administer medications, IV therapy, and treatments according to physician orders
- Assess and monitor patients' conditions, reporting changes to physicians and healthcare teams
- Collaborate with LPNs, CNAs, therapists, and other healthcare staff to develop and execute patient care plans
- Document patient assessments, care plans, and progress notes accurately
- Educate patients and families on disease management, rehabilitation, and wellness strategies
- Ensure compliance with state and federal regulations, facility protocols, and infection control policies
Qualifications
- Active Registered Nurse (RN) license in applicable state(s)
- Minimum 1 year of experience in a skilled nursing facility (SNF), long-term care, or rehabilitation setting
- Certification in Basic Life Support (BLS) required
- Knowledge of geriatric care, wound management, and chronic disease management preferred
- Strong communication, assessment, and critical thinking skills
