Responsibilities
- Assess patient needs and develop individualized care plans.
- Coordinate and facilitate continuity of care across multiple healthcare providers.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and families about health conditions, treatments, and resources.
- Ensure compliance with insurance, regulatory, and hospital policies.
- Advocate for patient needs and effective resource management.
- Maintain thorough documentation and case management records.
- Travel to different healthcare facilities as required for assignments.
Qualifications
- Education: Active Registered Nurse (RN) license.
- Experience: Minimum 2+ years in case management, discharge planning, or care coordination.
- Skills: Strong communication, problem-solving, and patient advocacy abilities.
- Certifications: BLS required; Case Management Certification (CCM) preferred.
- Travel Readiness: Willingness to accept travel assignments.
