Qualifications
- Education: Active Registered Nurse (RN) license.
- Experience: Minimum 2+ years in case management, discharge planning, or care coordination.
- Skills: Strong communication, problem-solving, and patient advocacy abilities.
- Certifications: BLS required; Case Management Certification (CCM) preferred.
- Travel Readiness: Willingness to accept travel assignments.
