Responsibilities
- Assess patients' medical needs and develop individualized care plans
- Coordinate patient care across different healthcare providers and settings

- Ensure compliance with healthcare regulations and insurance requirements

Qualifications
- Active Registered Nurse (RN) license in applicable state(s)
- Minimum 2 years of experience in case management or care coordination
- Case Management Certification (CCM) preferred but not required
- Strong organizational, problem-solving, and communication skills
- Experience with insurance authorizations, discharge planning, and resource allocation
- Proficiency in electronic health records (EHR) systems
- Willingness to travel and adapt to different healthcare settings
